Pick up almost any medicine box and the strength is printed in milligrams: 200 mg, 500 mg, 875 mg. Almost never in grams, even when the amount would convert to a tidy figure. That is not an accident or a convention nobody questioned — it is a deliberate safety choice.
The readability argument
Active ingredient quantities in medicine are small. Expressed in grams, most would be decimals — 0.5 g, 0.2 g, 0.875 g. Expressed in milligrams, the same quantities become 500, 200 and 875: whole numbers, no leading zeros, no decimal points.
That matters because a decimal point is the most dangerous character on a medical label. A smudged or missed point turns 0.5 into 5, a tenfold error. Written as 500 mg, there is no point to lose.

Why some labels still say grams
You will occasionally see a gram figure, most often at 1 g and above, where the milligram version starts to look unwieldy. A 1 g sachet is clearer than a 1,000 mg sachet, and bulk products like powders are frequently sold by the gram.
This is exactly where confusion between two packages arises. If a prescription says 0.5 g and the box in your hand says 500 mg, they describe the same amount — 0.5 × 1,000 = 500. Different manufacturers and different countries simply follow different labelling conventions.

Strength is not the same as dose
The single most useful distinction when reading a medicine label is between these two things:
- Strength — how much active ingredient is in one tablet, capsule, or measured amount of liquid. This is the mg figure printed on the box.
- Dose — how much a particular person should take, and how often. This comes from the instructions or a prescription, not from the strength alone.
Two 500 mg tablets contain 1,000 mg — one gram — of active ingredient between them. That is simple arithmetic and it is always true. Whether taking two is appropriate is an entirely separate question that the arithmetic cannot answer.
mg, mcg and the thousandfold trap
Alongside milligrams you will meet mcg — micrograms. There are 1,000 micrograms in a milligram, so mixing the two up misstates a quantity by a factor of a thousand. This is a genuinely serious error, and the labelling itself is designed to guard against it.
The SI symbol for microgram is µg, but medicine labels write mcg instead. The reason is handwriting: a hastily written µ can be read as an m, turning micrograms into milligrams. Spelling it mcg removes the ambiguity entirely.

The multiplication is trivial. What is not trivial is deciding whether that figure is correct for the individual, which depends on age, weight, the specific product and other medicines being taken. Work out the number if it helps you understand the instruction, then have it confirmed.
mg is not ml
On liquid medicines, two units appear side by side and they measure completely different things. Milligrams measure how much active ingredient is present. Millilitres measure how much liquid there is. A label reading "250 mg per 5 ml" means each 5 ml spoonful carries 250 mg of medicine.
There is no general conversion between them — it depends entirely on the concentration of that particular product. Two bottles of the same medicine at different concentrations will have different mg-per-ml figures, which is why the strength must always be read from the label rather than assumed.
A useful habit
